COOK, John David age 52 of West Carrollton passed away Sunday 
Sept. 15, 2013 following a sudden illness. John was preceded in death 
by his Father; John E. Cook, Paternal grandparents; Walter and Ethel 
Cook, Maternal Grandparents; Lillian and Theodore Thomas, Aunts 
and Uncles; Marion Haller, Carol Johnson, Raymond Cook and Jerry 
Cook. Surviving is his Mother; Loisnan Cook and sister Joni Cook both 
of Miamisburg, special aunts for their love, care and support during 
John's illness; Barb (Elmer) Marriott, Marie (Dick) Kurpe and Jane (Tom) 
Beachler, special friend Janet Hammond and family and numerous other 
relatives and friends. A memorial service to celebrate John's life will take 
place Saturday Sept. 21, 2013 at 4:00 p.m. at the Trinity Church of 
Miamisburg, 203 East Linden Avenue, Miamisburg, OH 45342 with Rev. 
Joe Getts officiating. In lieu of flowers please make contributions to the 
Humane Society of Greater Dayton, 1661 Nicholas Rd., Dayton, OH
